We’ll make the transition to any of our offices hassle-free. To save time before your next appointment, we invite you to fill out a medical history form and bring it when you next visit our offices. This form helps us to get to know you better, including any specific health concerns you might have, as well as how we can best meet your needs.


  • We recommend arriving to your appointment 10 minutes early in the event there are forms to complete. 
  • Our administrators offer appointment reminders via phone and email, and you are able to opt-out of this service at any time!
  • If you have insurance, please bring your insurance information to your first appointment. This will allow us to bill your insurance company directly!

Missed / Cancelled Appointments Policy

Since missed appointments and last minute cancellations impact our dentists’ schedule and our patients’ time and comfort, our office requires 48 hours notice to make changes to your appointment in order to avoid a cancellation fee. This advance notice allows our administrators to fill openings for patients looking to come in on short notice, and is a courtesy to our hard-working dental providers. We thank you for your understanding!
